About The Position

As a Hospice On‑Call RN, you’ll use your clinical expertise and compassionate approach to support patients and families during evenings, nights, weekends, or holidays. Your role ensures that every patient receives timely, comforting, high‑quality care aligned with their individualized plan of care. During your shift, you’ll be responsible for addressing the full scope of patient needs—providing hands-on care, education, assessment, and coordination as needed. You’ll work closely with the interdisciplinary team (IDG) and the Administrator on Call to ensure smooth communication, continuity of care, and meaningful support to patients and caregivers when they need it most.

Requirements

  • Active, current RN licensure.
  • Ability to meet all compliance requirements and maintain ongoing professional development.
  • Registered Nurse license in state of Iowa
  • Minimum 1 year acute care experience required
  • Must have reliable automobile, valid driver’s license, and the minimum state required liability automobile insurance
  • Frequent standing, walking, and lifting (about 35% of the time).
  • Ability to independently lift up to 35 lbs.
  • Ability to travel to patient homes or facilities.
  • You’ll help maintain a safe environment for patients, families, colleagues, visitors, and medical staff by following all safety standards, policies, and procedures.
  • All team members are expected to understand and uphold the organization’s Code of Ethics and Compliance Program.

Responsibilities

  • Direct Patient & Family Care Complete comprehensive assessments of physical, emotional, social, and spiritual needs.
  • Collaborate with patients, families, physicians, and the IDG to create and update the plan of care.
  • Deliver nursing interventions that maximize comfort, safety, and quality of life.
  • Provide education to patients and caregivers tailored to their learning needs.
  • Communication & Care Coordination Maintain ongoing communication with physicians, the Medical Director, the IDG, the Hospice Clinical Manager, and other partners involved in the patient’s care.
  • Coordinate follow‑up activities, medication needs, and visit schedules based on information from the Triage Nurse.
  • Document care promptly and thoroughly, ensuring accurate records that reflect the patient’s current condition and care goals.
  • Sync documentation after every visit to maintain real‑time accuracy of records.
  • On‑Call Responsibilities Respond to patient needs throughout the shift, including symptom management visits, routine visits, admissions, GIP visits, and Continuous Care (CC) visits.
  • Participate in on‑call assignments, routine rotations, and admissions as directed.
  • Work closely with the Administrator on Call (AOC) and Virtual Care Center (VCC) staff for coordination and support.
  • Attend IDG meetings, patient care conferences, and other collaborative meetings as needed.
  • Team & Organizational Support Participate in staff orientation, continuing education, and maintain all required licensure.
  • Follow all agency, state, and federal guidelines related to hospice and nursing care.
  • Take part in quality improvement activities such as peer review, huddles, and committee work.
  • Assist with chart audits, bypass activities, and other duties during downtime.
  • Complete any additional duties within the scope of your professional licensure.
