About The Position

Hillcrest Health & Living is seeking a Hospice RN to join their Hospice team in the Omaha area. The RN Case Manager will provide direct patient care services and manage patient care with specific knowledge and experience.

Requirements

• Must be licensed in the state of Nebraska or compact state as a Registered Nurse.
• At least three years of experience as a nurse.
• Must have a valid driver’s license.
• Must have a reliable means of transportation and carry current automobile insurance.
• BLS certification by AHA required.

Nice To Haves

• At least two years of experience in hospice or home care setting preferred.

Responsibilities

• Assess the physical, functional, psychosocial, and nutritional status of assigned patients in a systematic and continuous manner to provide required patient services.
• Professionally consults with physicians, other team members, patients/families, and other caregivers to formulation a plan of care and coordinate delivery of services.
• Provide nursing services in accordance with the plan of care and utilizing the nursing processes.
• Keep track of time spent daily on visits, travel, meetings, mileage, etc. and accurately place them into time spreadsheet.
