Requirements

Registered Nurse with valid CA state license
Minimum one year of hospice RN experience
Valid driver’s license, auto insurance, reliable transportation and willingness to drive to patients place of residence
Must have excellent interpersonal skills and a passion for hospice
Understanding of hospice philosophy and needs of the terminally ill
Self-directed with ability to work with little supervision
Current Physical and TB test results

Nice To Haves

Strong background in medical/surgical, ICU/CCU, or ER is helpful

Responsibilities

Perform RN duties on hospice team.
Serve as a RN Case Manager for the hospice Interdisciplinary Team
Assess for signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action
Establishes plan of care while collaborating and communicating with the Interdisciplinary Team including physician when appropriate
Coordinates all hospice services provided to patients/families in all settings to ensure quality of care
