About The Position

The position is for a part-time Nurse Clinician II in the WI Hospice Divisional - Administration, requiring 24 hours of work per week, specifically on weekends from 8 AM to 8 PM. The role involves providing and coordinating comprehensive patient care, utilizing the nursing process to achieve desired outcomes, and demonstrating a holistic approach to patient and family care. The nurse will develop therapeutic relationships, assess patient needs, and collaborate with healthcare professionals to ensure quality care.

Requirements

  • Current Illinois RN license.
  • Graduate from an accredited school of nursing.
  • Minimum 1 year of nursing experience.
  • Earn 10.0 continuing education credits annually.
  • Good oral and written communication skills.
  • Computer skills.
  • Good time management and organizational skills.

Nice To Haves

  • Bachelor's Degree in Nursing.
  • Demonstrated proficiency for the requirements for NC I.

Responsibilities

  • Develops and maintains a therapeutic relationship throughout the healthcare continuum.
  • Incorporates responsibility, accountability, clinical decision-making, authority, shared governance, and critical thinking in daily nursing practice.
  • Provides individualized care to promote physical and psychosocial comfort.
  • Assesses patient and family readiness and involves them in planning and implementing care.
  • Incorporates an individualized, holistic approach in the plan of care.
  • Continually evaluates patients' and significant others' response to interventions and modifies the plan of care as needed.
  • Collaborates with physicians and other healthcare professionals in developing care plans.
  • Supports patients' decisions regarding treatment options.
  • Provides supportive care to family members throughout the patient's illness.
  • Facilitates the patient's right to receive quality, cost-effective care.
  • Participates in quality/process improvement initiatives.
  • Demonstrates commitment to meeting the learning needs of patients and families.
  • Plans, implements, and provides follow-up education for patients and significant others.
  • Recognizes changes in clinical situations and implements changes to the plan of care.
  • Demonstrates effective communication and conflict resolution skills.
  • Participates in unit meetings and activities that improve patient care.
