About The Position

Join James River Home Health and Hospice as a PRN Registered Nurse , providing one-on-one patient care in the home setting. This role focuses on individualized care, care plan management, and supporting patients and families as part of a collaborative interdisciplinary team. PRN coverage will be based between the Farmville and Lynchburg areas, depending on patient needs. What’s in it for Registered Nurses? Provide skilled nursing care and treatments as prescribed. Educate patients/families on end-of-life care, healthcare decisions, and dietary counseling. Conduct death visits and provide bereavement support as needed. Administer medications and stay updated on drug therapies. Case Management: Coordinate all aspects of patient care, including services provided by IDG and contracted providers. Ensure continuity of care by managing patient schedules and collaborating with team members to address emerging needs. Keep patient care plans up-to-date and ensure documentation is accurate, complete, and timely Supervise and evaluate care provided by Hospice Aides and Licensed Practical Nurses (LPNs), conducting supervisory visits as required

Requirements

  • Must be a Registered Nurse with a current license to practice nursing in the Commonwealth of Virginia
  • At least one year of hospice experience is required.
  • Graduate of an accredited nursing school.
  • Must have a valid driver's license and a reliable car available for use during work hours.
