About The Position

The Hospice RN On Call Weekends plays a critical role in providing compassionate, patient-centered care to individuals facing life-limiting illnesses during weekend hours. This position ensures continuity of care by responding promptly to patient and family needs, managing symptoms, and coordinating with interdisciplinary teams to uphold the highest standards of hospice care. The RN will serve as a vital resource for patients, families, and healthcare providers, offering clinical expertise and emotional support during challenging times. This role requires the ability to make independent clinical decisions while adhering to hospice policies and regulatory requirements. Ultimately, the Hospice RN On Call Weekend contributes to enhancing the quality of life for patients and supporting their families through skilled nursing care and empathetic communication.

Requirements

  • Current and valid Registered Nurse (RN) license in the state of practice within the United States.
  • Minimum of two years of nursing experience, preferably in hospice, palliative care, or a related clinical setting.
  • Basic Life Support (BLS) certification.
  • Strong clinical assessment and critical thinking skills.
  • Ability to work independently and make sound clinical decisions during on-call hours.

Nice To Haves

  • Certification in Hospice and Palliative Nursing (CHPN) or equivalent specialty certification.
  • Experience with HOMECARE HOMEBASE EMR
  • Previous experience working in an on-call nursing role or similar remote clinical support position.
  • Demonstrated skills in patient and family education and emotional support.

Responsibilities

  • Respond promptly to patient and family calls during weekend on-call shifts, providing clinical guidance and support.
  • Assess patient symptoms and needs remotely or through home visits as necessary, implementing appropriate interventions.
  • Coordinate with interdisciplinary hospice team members, including physicians, social workers, and chaplains, to ensure comprehensive care.
  • Document all patient interactions, clinical assessments, and interventions accurately and timely in the electronic medical record system.
  • Provide education and emotional support to patients and families regarding symptom management, medication use, and end-of-life care.
  • Ensure compliance with hospice policies, procedures, and regulatory standards during all on-call activities.
  • Escalate urgent clinical issues to the appropriate healthcare providers or emergency services when necessary.
