About The Position

Hospice Registered Nurse (RN) Liaison - Capital Region -Primarily Ellis, St. Peter's Hospital, and Samaritan Hospitals The Community Hospice has a rewarding opportunity for a RN to work as a part of our Hospice Intake RN Team serving as a Liaison and Hospice RN in local hospitals (Capital Region-primarily Ellis, Samaritan, and St Peters hospitals) Position Highlights: NO WEEKENDS, NO ONCALL National pre-taxed mileage reimbursement Meetings and charting conducted via supplied lap top Comprehensive orientation provided Full benefits through St Peters Health partners What you will do: Evaluates patients for eligibility and interest in hospice services, describes the support and care available from hospice along with the benefits of hospice care. Coordinates with the hospital providers and our hospice medical directors to confirm eligibility for hospice services. Gathers information and ensures a smooth handoff to the hospice RN admitting the patient/family to services. Admits to hospice inpatient services upon eligibility, develops a hospice plan of care and collaborates with the hospice interdisciplinary team and the acute care team. Works closely with the hospital case management department. The work schedule is flexible, generally weekdays (M-F) between 8am to 4:30pm. What you will need: A current license to practice as a Registered Nurse in the State of New York Associates Degree in Nursing, BSN preferred 1 year experience as an RN required, preferably in Hospice/Home Care or acute care. Hospice or RN Liaison experience a plus but not required. The RN must be able to communicate effectively, verbally and written Valid Driver's License and Reliable vehicle High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers Understanding of Hospice principles preferred Training provided in end of life care. Pay Range: $38.00-$50.18 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location. Requirements

  • A current license to practice as a Registered Nurse in the State of New York
  • Associates Degree in Nursing, BSN preferred
  • 1 year experience as an RN required, preferably in Hospice/Home Care or acute care.
