Hospice RN (Hospital Based) – Acute Symptom Management & Admissions/Transitions of Care
About The Position
This is hospice nursing at the highest level. Granite VNA Hospice is seeking an experienced Hospice RN to provide expert hospice admissions and acute, inpatient-level symptom management within a hospital setting. This role supports patients and families at critical moments, including those who require Hospice House–level care, delivered in hospital or alternative settings when inpatient hospice placement is not immediately available.If you are a nurse who values clinical autonomy, interdisciplinary collaboration, and the privilege of guiding patients and families through complex end-of-life decisions, this role offers meaningful, high-impact work every day. Requirements
- Licensed RN in New Hampshire (or eligible)
- Minimum 2 years of RN experience (Hospice, palliative care, ICU, ED, oncology, or acute care preferred)
- Strong clinical assessment, communication, and critical-thinking skills
- Comfort supporting patients and families through emotionally complex situation
