Worth Metroplex Role Summary The Hospice RN – Float provides flexible, skilled nursing support across hospice service lines to meet patient and organizational needs. In this PRN role, the nurse may support admissions, case management, and after-hours care, delivering timely clinical assessments, symptom management, and compassionate guidance to patients and families. Serving the DFW Metroplex, the Hospice RN – Float collaborates closely with the interdisciplinary team to ensure continuity of care, regulatory compliance, and a high-quality hospice experience across settings. Overview The Hospice RN - Float provides skilled nursing care across hospice case management, admissions, and after-hours services, flexing to meet organizational and patient care needs. This role delivers clinical evaluations, care planning, urgent interventions, and ongoing support to patients and families, ensuring seamless, compassionate hospice care across all service lines.

Requirements

  • Graduate of an accredited nursing program; Associate’s degree or higher preferred.
  • Current RN license in practicing state or a valid multistate compact license.
  • Minimum one (1) year of hospice, palliative care, or home health experience preferred.
  • CPR certification and valid driver’s license required.
  • Strong clinical judgment, documentation accuracy, and communication skills.
  • Knowledge of hospice regulations and Medicare Conditions of Participation preferred.
  • Ability to work independently while collaborating across interdisciplinary teams.
  • Strong communication, critical thinking, and documentation skills.
  • Excellent communication, change management, and project execution skills are required.

Responsibilities

  • Provide flexible nursing support across case management, admissions, and after-hours roles based on organizational needs.
  • Conduct clinical assessments, evaluate patient eligibility, and complete admissions including consents and initiation of hospice services.
  • Manage a hospice caseload when assigned, including symptom management, care coordination, and interdisciplinary collaboration.
  • Respond to urgent patient and family needs during after-hours or on-call shifts, including symptom control, medication coordination, and death pronouncements.
  • Coordinate care with attending physicians, facility staff, and other providers to ensure aligned, goal-concordant hospice care.
  • Provide emotional support, education, and guidance to patients, families, and caregivers during all phases of care.
  • Ensure accurate, timely documentation to support clinical, regulatory, and billing compliance.
  • Maintain current knowledge of hospice eligibility, services, and regulatory requirements.
  • Participate in staff meetings, training, and quality improvement activities as assigned.
  • Perform other duties as assigned.
