Requirements

• Graduate of an approved nursing education program.
• Licensure as a Registered Nurse in the State of Pennsylvania.
• 2 years experience as a professional nurse in acute care, home care or ER experience required.
• Home care or Hospice experience preferred but not required.
• EPIC experience preferred.

Responsibilities

• Responsible for case managing a team of hospice patients.
• Collaborate with interdisciplinary team and attend weekly Interdisciplinary meetings.
• Interact with the primary care physician and/or hospice medical director as needed to effectively manage patient symptoms.
