Apply now and let's soar together at Blue Skies of Texas! As a Hospice RN Case Manager, you will be responsible for providing skilled professional nursing care to the patient and family. The incumbent oversees quality of care delivery by all disciplines and programs, assures delivery of optimal care safely and effectively, and identifies both existing and potential problems that impact the quality of patient care. Also ensures that provision of the highest quality of care to residents receiving Hospice services. This role is critical in directing, assessing, planning, implementing, and overseeing the care provided to patients. The incumbent occupies a position of confidence and trust and must maintain confidentiality in accordance with Health Insurance Portability and Accountability Act regulations.

Requirements

  • Bachelor of Science in Nursing (BSN) degree is preferred.
  • 1 3 years of direct experience with skilled nursing community admissions and demonstrate comprehension of Medicare, Managed Care and private insurance requirements for admission to a skilled nursing community is required.
  • You must possess working knowledge of Health Insurance Portability and Accountability Act guidelines.
  • Must possess a current, valid Texas drivers license with good driving record, remain insurable by Village carriers, and apply for and obtain a military base pass within six weeks of employment is required.
  • Required: Registered Nurse license in good standing with the state of Texas or a compact state allowing practice in Texas.
  • Required: Current certifications in: Cardiopulmonary Resuscitation (CPR), First Aid, and Automated External Defibrillator (AED).
  • Required: Valid Texas driver’s license with a good driving record; must remain insurable under Blue Skies carriers.
  • At least 1-3 years in clinical nursing for Hospice care with knowledge of acute and chronic disease management
  • Working knowledge of computer skills, including Microsoft Office (Word, Excel, PowerPoint).

Nice To Haves

  • A bachelor’s degree in Nursing, as well as at least one year of clinical nursing experience—preferably in hospice—with a focus in adult or geriatric nursing, acute and chronic disease management, patient teaching, and phlebotomy and intravenous skills.
  • Certification in Hospice.

Responsibilities

  • Provides patient care in accordance with accepted standards of nursing practice, agency policies and procedures, and the written plan of care.
  • Assists the case manager with implementing and coordinating patient care through teamwork and the efficient use of available resources.
  • Creates clinical solutions to patient problems utilizing the nursing process and critical thinking skills.
  • Reinforces teaching initiated by the Registered Nurse (RN).
  • Collaborates with various healthcare professionals to determine the best care for assigned patients.
  • Communicates schedule to case manager, supervisors, and other team members through established processes.
  • Keeps current with trends and advancements of home health and hospice care.
  • Participates in the interdisciplinary team conferences as directed.
  • Participates in ongoing quality assurance and performance improvement activities as assigned.
  • Participates in staff development: attending in-service and continuing education programs to maintain proficiency
  • Completes documentation according to agency policies and procedures.
  • Reports changes in client’s condition to the RN.
  • Performs other duties as directed.
