We’re currently seeking a full‑time Hospice RN Case Manager to serve patients in the Towanda, PA area. Your Role in Making a Difference As a Hospice RN Case Manager, you’ll be the clinical lead for your patients, coordinating and delivering high‑quality, compassionate end‑of‑life care in collaboration with physicians, social workers, aides, chaplains, and the broader interdisciplinary team. Position Summary: As a member of the interdisciplinary team, the Hospice Nurse works under the general direction of the Director of Hospice and the attending physician in the provision of skilled nursing assessment, planning and care in order to maximize the comfort and health of patients and families. Responsible for identifying patient/family needs and for providing supportive care in accordance with the attending physician’s orders and interdisciplinary plan of care and Guthrie Hospice policies and procedures. Responsible to provide direct supervision of Home Health Aides and or LPN’s providing care for Guthrie Hospice.
