We are seeking a Hospice Nurse Case Manager to be responsible for identifying patient/family needs and for providing skilled nursing assessment, planning and supportive care and appropriate services in accordance with the attending physician’s plan of treatment and orders. Schedule: Monday - Friday; 8:30am - 5:00pm Location: Alameda and Contra Costa Counties Hope Hospice follows all CDPH vaccine requirements for healthcare personnel.

Requirements

  • One year experience as a professional nurse in the past three years or Bachelor’s degree in Nursing from an accredited School of Nursing.
  • Current license as a registered professional nurse in the State of California.
  • Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.

Nice To Haves

  • Experience in acute care/ home health and/or hospice setting preferred
  • Public Health Nursing certification highly desirable.
  • HPNA certification preferred.

Responsibilities

  • Assume primary responsibility for a patient/family caseload that includes the assessing, planning, implementing and evaluating phases of the nursing process.
  • Maintain a caseload and productivity per agency’s protocol.
  • Initiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family.
  • Maintain regular communication with Director of Patient Services/designee to review caseload.
  • Maintain regular communication with the attending physician concerning patient/family care needs and response to interventions.
