About The Position

The Hospice RN Case Manager coordinates and manages the care of terminally ill patients, ensuring they receive comprehensive and compassionate support throughout their end-of-life journey.

Requirements

  • Completion of an accredited Registered Nurse (RN) program.
  • Current RN license in the state of practice.
  • Must possess and maintain current CPR certification.
  • Strong clinical and patient care skills.
  • Excellent interpersonal and communication abilities.
  • Proficiency in electronic medical records (EMR) systems and office software.
  • Ability to work independently and as part of an interdisciplinary team.
  • Compassionate, empathetic, and dedicated to providing high-quality patient care.
  • Ability to lift up to 75 pounds and turn, move, and transport patients using proper body mechanics and lifting devices as needed.
  • Capacity to stand, walk, and sit for extended periods.
  • Ability to perform repetitive tasks and physical activities, such as bending, kneeling, and reaching.
  • Work is performed in a healthcare setting with potential exposure to infectious diseases, bodily fluids, and chemicals.
  • Must be able to work various shifts, including weekends, holidays as needed.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  • Must be able to travel throughout the service area.

Nice To Haves

  • Certification in Hospice and Palliative Care Nursing (CHPN) preferred.
  • Previous experience in hospice or palliative care preferred.

Responsibilities

  • Conduct initial and ongoing assessments to evaluate patients’ physical, emotional, and spiritual needs.
  • Develop personalized care plans in collaboration with the patient, family, and interdisciplinary team.
  • Review and update care plans based on changes in patient condition and goals of care.
  • Serve as the primary point of contact for patients, families, and caregivers.
  • Coordinate services and resources to ensure continuity of care.
  • Facilitate communication among healthcare providers and community agencies involved in patient care.
  • Maintain accurate and up-to-date clinical records, including patient assessments, care plans, and progress notes.
  • Ensure compliance with regulatory standards and hospice policies.
  • Participate in interdisciplinary team meetings to discuss patient care plans and progress.
  • Collaborate with other team members, including nurses, physicians, social workers, and chaplains, to provide holistic care.
  • Educate patients and families about disease progression, symptom management, and end-of-life care options.
  • Provide emotional support and guidance to help patients and families cope with grief and loss.
  • Monitor and manage symptoms to promote comfort and quality of life.
  • Coordinate with physicians and other healthcare providers to adjust medications and treatments as needed.
  • Follow infection control guidelines and maintain privacy rules and regulations as per HIPAA.
  • Attend all meetings and complete in-services and mandatory training.
  • Provide exceptional customer service and high-quality care by showing compassion and empathy towards patients and their families.
  • Performs other related duties as assigned
