About The Position

Life Care Home Health Family provides skilled nursing, therapy, homecare, hospice, palliative care, and private duty services across Texas, Florida, Nevada, Georgia, and Arizona. The RN Case Manager performs admissions and administers nursing care for patients of all ages in their place of residence. This role involves coordinating care with the Interdisciplinary Group/Team, patient/family, and referring agencies, and assuming overall responsibility for care coordination.

Requirements

  • Two (2) years experience as a Registered Nurse in a clinical care setting
  • One (1) year full time hospice experience
  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
  • Current State license as a Registered Nurse
  • Current driver’s license
  • Reliable transportation and valid auto liability insurance
  • Nursing skills as defined as generally accepted standards of practice
  • Knowledge of state and federal regulations
  • Good interpersonal skills

Responsibilities

  • Assess hospice patients utilizing the Nursing Assessment and LCD criteria to identify their physical, psycho-social, and environmental needs
  • Develop and document the plan of care in correlation with the Assessment
  • Provide nursing care utilizing infection control measures (OSHA)
  • Work with Interdisciplinary Group/Team to ensure patient/family needs are met
  • Communicate with attending physicians, hospice physicians, and other physicians involved in the patient’s care
  • Timely submission of all required paperwork
  • Demonstrate commitment and professional growth by participating in hospice-sponsored in-service programs and maintaining or improving competency
  • Participate in the Hospice’s QAPI program
  • Promote the Hospice’s philosophy and administrative policies
  • Meet productivity standards
  • Perform on-call responsibilities and provide on-call services to patients and their families as assigned
  • Provide effective communication to patients, their family members, team members, and other health care professionals
