About The Position

At Trinity Hospice, RN Case Managers build meaningful relationships with patients and families, focusing on comfort, dignity, and quality of life during important moments. The role offers autonomy, purpose, and a manageable caseload within an elite clinical team. A typical day involves visiting patients in their homes, assessing needs, managing symptoms, adjusting care plans, coordinating with other healthcare professionals (physicians, hospice aides, LPNs, social workers, chaplains), supporting and educating families, and documenting care efficiently in the EMR (Consolo). RNs advocate for patients while maintaining autonomy in structuring their day. Trinity Hospice emphasizes taking care of its nurses, offering flexibility, support, and respect.

Requirements

• Active RN license in the state you’ll practice
• Graduate of an accredited nursing program (ADN or BSN)
• Strong clinical judgment, compassion, and communication skills
• Reliable transportation
• Valid driver’s license
• Auto insurance
• CPR certification

Nice To Haves

• 2+ years of RN experience preferred
• Hospice experience a plus

Responsibilities

• Serve as the primary nurse coordinating care for your assigned patients
• Develop, implement, and update individualized plans of care
• Determine visit frequency based on patient acuity and family needs
• Provide guidance to Hospice Aides and LPNs to ensure excellent care
• Communicate changes in condition to physicians and the care team
• Participate in on-call rotation
