Requirements
- Graduate of an accredited nursing program
- Active RN license in the State of Iowa
- Minimum 1 year of professional nursing experience (acute care preferred)
- Valid driverâs license and reliable transportation
Nice To Haves
- Hospice or home care experience preferred
- Strong communication and interpersonal skills
Responsibilities
- Develop and manage individualized care plans
- Provide symptom management and emotional support
- Educate patients and families on end-of-life care
- Coordinate with physicians and interdisciplinary teams
- Supervise Hospice Aides and LPNs
- Document care accurately using point-of-care technology
- Participate in IDT/IDG meetings and case conferences
- Ensure timely communication and care coordination
