About The Position

Make a Lasting Difference as a Hospice Registered Nurse Case Manager (RNCM)! Are you a compassionate Registered Nurse (RN) who finds purpose in supporting patients and families through meaningful moments of care? Hospice of the West is seeking a Tucson Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community. Schedule: Monday - Friday, 8 am - 5 pm with an hour lunch break. Back Up On Call Every 6th Weekend 8 am - 5 pm and Holiday Rotation. Territory: Northern Oro Valley and Surrounding Area. In this rewarding role, you’ll deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. Requirements

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting.
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.
