$10,000 sign on bonus with benefits DAY 1! Coverage Region: Hartford Schedule is M-F, 8a- 5p with one weekend every 4 weeks and typically 2-3 holidays a year that are rotated This position is 100% home care providing services in patients private homes. Hospice RN Case Manager position summary Our hospice RNs use cutting edge technology and clinical knowledge to provide exceptional, compassionate care to patients with life-limiting or terminal illnesses in their homes. Hospice nurses are responsible for patient assessment, symptom control and overall supervision of patient care and outcomes.
