Hospice RN Case Manager

Flexible, full-time positions available in the East Valley (serving Tempe, Mesa, Chandler, Apache Junction, and Gilbert areas) OR Central/Southern Phoenix (serving Peoria, South Mountain, and Laveen areas). $80000 - $90000 annual salary, depending on experience. Travel expenses paid, company phone and laptop provided!
Requirements
- Current RN license in the State of Arizona
- Minimum of 2 years of nursing experience, hospice or home health experience preferred
- Valid driver’s license and reliable, insured vehicle
- Strong clinical skills and a passion for patient-centered care
- Ability to work independently and a passion for making a real difference
Nice To Haves
- Homecare Homebase (HCHB) experience preferred, not required
Responsibilities
- Complete comprehensive assessments of patients and families to determine hospice needs
- Develop and implement individualized care plans, revising as needed
- Provide skilled nursing care, administer medications, and manage symptoms
- Educate patients and families on care techniques, medications, and comfort measures
- Maintain accurate clinical documentation and communicate changes to physicians
- Participate in interdisciplinary team meetings and quality improvement initiatives
- Provide emotional support and counseling to patients and families
- Supervise hospice aides and delegate responsibilities appropriately
- Collaborate with an interdisciplinary team that shares your commitment to excellence
- Participate in holiday on-call rotation as needed
