Job Description Full-time Hospice RN Case Manager position Schedule: Monday thru Friday, 8a-5p Available territory openings: Case Manager typically assigned to cover 1-2 counties near the Office. Performs physical and psychosocial assessment on all hospice patients. Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management. Responds to the physical, mental, social and emotional needs of terminally ill patients and their families. Assesses patientâs condition, initiates plan of care, re-evaluates and updates as necessary. Obtains all necessary physician orders and collaborates with physician as patientâs needs dictate. Initiates case conferences to discuss multidisciplinary team responsibilities, pain management, bereavement counseling, etc We hope to speak with you about the rewarding nature of hospice nursing! Requirements

One year Hospice experience preferred
2 years of RN experience required
Licensed Registered Nurse in State of Indiana
Current driver's license
Must have reliable transportation and insurance

Responsibilities
