Requirements
- A compassionate, dedicated RN Case Manager who makes patients feel at home—whether they’re in their own home, an assisted living facility, or a skilled nursing home.
- Hospice or home health Jedi with a valid GA RN license
- Driver’s license
- At least one year of experience
Responsibilities
- Be a steady source of comfort and support to patients and families
- Help families understand disease progression, self-care, and nutrition
- Collaborate with the interdisciplinary hospice team to create and follow patient care plans
- Manage symptoms like a boss and advocate for patient comfort
- Maintain timely and accurate clinical documentation
