The Registered Nurse (RN) Case Manager (PRN) will provide comprehensive care coordination and support to hospice patients and their families. The RN Case Manager will be responsible for assessing patient needs, developing individualized care plans, coordinating care services, and providing emotional support throughout the end-of-life journey.

Requirements

  • RN licensure for the state of Ohio
  • Clean driving record, and valid driver’s license and auto insurance
