About The Position
Varco Hospice and Home Health is seeking a compassionate and experienced RN Case Manager to join our dedicated team of clinicians in Southwest Houston. This full-time position offers a hybrid work environment, allowing for flexibility while ensuring that our patients receive the highest quality of care. The RN Case Manager will be responsible for managing the care of patients in their homes, ensuring compliance with agency policies and Medicare/Medicaid regulations. The ideal candidate will have a strong background in hospice care and be proficient in using Kinnser/WellSky software for patient management. In this role, the RN Case Manager will coordinate and supervise the delivery of care provided by LPNs and HHAs, ensuring that all patient care is executed according to established plans. The RN will perform essential tasks such as admissions, transfers, re-certifications, and discharges, utilizing Oasis documentation to maintain accurate clinical records. Education is a key component of this position, as the RN will be responsible for informing patients and their caregivers about disease processes, medications, and individualized treatment plans. The RN will also supervise LVNs and HHAs, notifying the physician and other relevant personnel of any changes in the patient's condition. The RN Case Manager will work Monday to Friday during the day shift, with on-call responsibilities and the potential for weekend work as needed. This position requires a valid RN license and a minimum of one year of experience in case management, field nursing, and in-home hospice care. The successful candidate will be expected to relocate to the Houston area before starting work, as the role involves servicing various locations throughout the city.
