About The Position

The Hospice Registered Nurse (RN) Case Manager provides professional nursing care to hospice clients as prescribed by the physician and in compliance with the state’s Nurse Practice Act and the organization’s policies. The RN is responsible for client assessment, care planning, therapeutic intervention, and overall supervision of client care and outcomes. The RN collaborates with the physician and other team members to manage patient symptoms and assist in grief/loss issues, implementing the Hospice plan of care. The position is based in the Hartford region.

Requirements

• Graduate of an approved nursing education program.
• Licensure as a Registered Nurse in the State of Connecticut.
• One (1) year experience as a professional acute care nurse.
• Must have a current Driver’s license and reliable transportation.

Nice To Haves

• Hospice home care experience preferred but not required.

Responsibilities

• Maintains knowledge of applicable Federal, State, and local laws and regulations, and adheres to the Organizational Integrity Program and Code of Ethics.
• Informs clients/caregivers about the agency, care/services available, and client obligations before care initiation.
• Develops and implements an individualized Plan of Care with realistic and attainable goals.
• Makes appropriate referrals for evaluation/care to other disciplines and coordinates care with others.
• Utilizes interview, observation, and evaluation in assessing clients and applies nursing judgment in formulating interventions.
• Reports changes in client condition to the physician and obtains orders for changes in treatment.
• Re-evaluates and updates the patient’s plan of care based on goals and progress.
