About The Position

The Hospice RN Case Manager is a professional nurse who coordinates and directs the hospice patient's care in their home environment (private residence, skilled nursing facility, assisted living, hospital, etc.) based on individual needs. The Hospice RN Case Manager is responsible for independent management of the Hospice patient population requiring use of advanced assessment, teaching and decision-making skills. The Hospice RN Case Manager cares for a caseload of hospice patients by evaluating the patient for appropriateness of hospice care and developing the plan of care in collaboration with the provider, caregivers and Hospice IDG members. The Hospice RN Case Manager provides follow up by evaluating the effectiveness of the plan of care and monitoring the patient and caregivers response to the plan. The Hospice RN Case Manager provides patient care based on provider's orders, departmental policies and procedures, regulatory guidelines and best practice. The Hospice RN Case Manager documents in the patient's medical record in a thorough and timely manner according to agency requirements. The Hospice RN Case Manager attends and actively participates in IDG and team meetings. The Hospice RN Case Manager performs on-call responsibilities and Holiday staffing.

Requirements

  • Graduate from an accredited Diploma of Nursing Program or Associate's Degree in Nursing.
  • Current Texas state license in appropriate discipline.
  • Current American Heart Association CPR certification.
  • Current Texas state driver's license and auto insurance.
  • Two years' experience in a healthcare setting in respective discipline.

Nice To Haves

  • Bachelor's Degree in Nursing.
  • Hospice and Palliative Care certified.
  • Wound Care Certified.
  • Advanced education or certification in area of specialty.
  • Home Care Home Base experience.
  • Experience with smart phones and smart devices.
  • Medical-Surgical experience.
  • Experience in Case Management.
  • Supervisory position in healthcare setting.
  • Home Care or Hospice experience.

Responsibilities

  • Coordinate and direct hospice patient care in various home environments.
  • Manage a caseload of hospice patients and evaluate their appropriateness for hospice care.
  • Develop and implement a plan of care in collaboration with providers and caregivers.
  • Evaluate the effectiveness of the plan of care and monitor patient responses.
  • Document patient care in medical records according to agency requirements.
  • Participate in IDG and team meetings.
  • Perform on-call responsibilities and holiday staffing.
