The Admissions Manager is responsible for the patient assessment and administrative processes and admit patients to By the Bay Health service. This includes information interviews with patients and families, review of eligibility for the Hospice benefit, physician orders, patient assessment, processing of regulatory and organizational documents. The manager is responsibility for achieving organizational metrics. The position is expected to build and maintain optimal relationships with all referral sources in compliance with organization policies and procedures and applicable laws and regulations Schedule: Monday - Friday; 8:30am - 5:00pm; AOC duties required Hybrid - 3 days required onsite Location: Larkspur By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.
