About The Position

AdvisaCare is hiring a Hospice Registered Nurse (RN) to provide one-on-one, compassionate, and personal care to patients in the comfort of their homes. This role focuses on supporting patients and families with dignity and heart during their final days, emphasizing gentle and deeply meaningful care without the chaos of hospital settings. AdvisaCare is a growing hospice agency based out of Troy, servicing clients in Portage and surrounding areas. The ideal candidate is a compassionate, dynamic, energetic, organized, and efficient professional with hospice experience who values hospice care. Responsibilities include providing emotional support, conducting supervisory visits for team members, and participating in interdisciplinary group meetings for patient care planning.

Requirements

Registered nurse who sees the value of Hospice care
Current Michigan License in good standing
Minimum 1 year hospice experience
Empathetic approach to patients and caregivers
Participate in field visits daily, weekly office meetings as indicated
Ability to adapt to patient’s status and needs on a full time on call basis
Position requires On-Call responsibilities!
Rotating Schedule

Responsibilities

Provide one-on-one care in the comfort of home
Support patients and families with dignity and heart
Care without the chaos of hospital halls
Care the way it’s meant to be—gentle, personal, and deeply meaningful
Conduct appropriate supervisory visits, which allow for clinical observation of the performance of assigned team members
Participate and provide feedback to the team for regular interdisciplinary group meetings, on the development, review, and revision of the patient/family plan of care
Initial and on-going assessment of patient’s physical, functional, psychosocial and environmental needs
Implement the individualized plan of care and revisions as needed
Compliance with AdvisaCare Hospice Policies and Procedures, Medicare compliance
Consulting with and educating the patient/family on disease process, self-care techniques, end-of-life care, dealing with issues of ethical concern
Supervision of paraprofessionals providing services to patient
Preparing clinical and progress notes, participating in IDG conferences
Participate in Hospice performance improvement program
Maintain high level of customer satisfaction with our clients, families and caregivers
Communication of information using current process and technology within organization
