About The Position

At St. Croix Hospice, Registered Nurses guide patients and families through the end-of-life journey by providing compassionate care focused on the patient's quality of life, empowering them to make the most of their time with dignity, comfort, and respect. The Registered Nurse is experienced in nursing and provides day-to-day care for hospice patients based on their specific Plan of Care under the direction of the patient’s RN Case Manager. This role focuses on providing comfort and quality of life, managing treatment, administering medications, and maintaining patient records.

Requirements

  • Graduate of an accredited school of nursing required.
  • Associate degree required.
  • Must have a current Registered Nurse (RN) license in state(s) of practice.
  • Ability to pass background, consumer report and drug test.
  • Valid State Driver’s License, access to an automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record required.

Nice To Haves

  • 1+ years management experience, preferably in a hospice/home care setting preferred.
  • 2+ years' experience in Nursing, preferred

Responsibilities

  • Perform all duties and responsibilities in accordance with the Nurse Practice Act and in accordance with basic principles and guidelines of professional nursing.
  • Complete initial and ongoing comprehensive assessment of patient and family to determine hospice needs.
  • Provide a complete physical assessment of all current and previous health history.
  • Evaluate patient status by writing and initiating Plan of Care, regularly re-evaluating patient and family/caregiver needs, and participating in Plan of Care revision as necessary.
  • Use health assessment data to determine nursing diagnosis.
  • Develop a plan of care that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions, including patient and family in the planning process.
  • Initiate appropriate preventive and rehabilitative nursing procedures.
  • Administer medications and treatments as prescribed and determined by the physician’s plan of care.
  • Provide health care instructions to the patient as appropriate per assessment and plan.
  • Complete, submit, and maintain accurate and relevant clinical notes regarding patient’s condition and care given.
  • Record pain/symptom management and changes/outcomes as appropriate.
  • Communicate with the physician regarding patient needs and report changes in patient condition; obtain/receive physician orders as required.
  • Assist the patient with the activities of daily living and facilitate the patient’s efforts toward self-sufficiency and optional comfort care.
  • Provide and maintain a safe environment for the patient.
  • Counsel and work in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver.
  • Teach the patient and family/caregiver self-care techniques as appropriate.
  • Provide medication, diet and other instructions as ordered by the physician.
  • Recognize and utilize opportunities for health counseling with patients and families/caregivers.
  • Work in concert with an interdisciplinary care team.
  • Coordinate Plan of Care with relevant community health partners.
  • Ensure arrangements for equipment and other necessary items and services are available.
  • Maintain compliance with all HIPAA regulations and requirements.
  • Treat all patient information as confidential and utilize protected health information on a minimum necessary basis.
  • Participate in on-call duties per policy.
  • Attend interdisciplinary group meetings.
  • Supervise and delegate tasks to ancillary personnel as needed.
  • Engage in continuing education and professional development.
  • Contribute to quality assessment and performance improvement activities.
  • Maintain a calm and professional demeanor in patient homes.
  • Perform other duties as assigned.
