Requirements

  • Hold a current Arizona RN License in good standing.
  • Have at least 1 year of hospice experience.
  • Exhibit empathy and a compassionate approach towards patients and their families.
  • Are able to adapt to the evolving needs of patients on an on-call basis.
  • Will attend weekly office meetings and complete field visits daily.

Responsibilities

  • Conducting thorough initial and ongoing assessments of each patient's needs—physical, psychosocial, and environmental.
  • Implementing and adjusting individualized care plans, in collaboration with the healthcare team, to meet the dynamic needs of our patients.
  • Ensuring compliance with all AdvisaCare Policies and Procedures , including Medicare standards.
  • Providing education and support to patients and families on illness, self-care techniques, and end-of-life matters.
  • Supervising paraprofessionals and offering guidance on patient care.
  • Documenting clinical progress notes and participating in interdisciplinary team conferences.
  • Engaging in our performance improvement programs to elevate the quality of care.
  • Promoting high satisfaction among clients, families, and caregivers, ensuring they feel valued and supported.
  • Utilizing current technology to communicate necessary information within the organization.
