Requirements

Hold a current Arizona RN License in good standing.
Have at least 1 year of hospice experience.
Exhibit empathy and a compassionate approach towards patients and their families.
Are able to adapt to the evolving needs of patients on an on-call basis.
Will attend weekly office meetings and complete field visits daily.

Responsibilities

Conducting thorough initial and ongoing assessments of each patient's needs—physical, psychosocial, and environmental.
Implementing and adjusting individualized care plans, in collaboration with the healthcare team, to meet the dynamic needs of our patients.
Ensuring compliance with all AdvisaCare Policies and Procedures , including Medicare standards.
Providing education and support to patients and families on illness, self-care techniques, and end-of-life matters.
Supervising paraprofessionals and offering guidance on patient care.
Documenting clinical progress notes and participating in interdisciplinary team conferences.
Engaging in our performance improvement programs to elevate the quality of care.
Promoting high satisfaction among clients, families, and caregivers, ensuring they feel valued and supported.
Utilizing current technology to communicate necessary information within the organization.
