Our Hospice Care team is seeking a dedicated Registered Nurse to deliver personalized, one-on-one care in the comforting setting of patients' homes.

Requirements

  • Vermont Licensed RN with a passion for hospice and palliative care.
  • Valid VT driver’s license and reliable vehicle with automobile insurance is essential for home visits.

Nice To Haves

  • Prior hospice experience and/or case management background preferred, though comprehensive training and orientation will be provided.

Responsibilities

  • Provide compassionate end-of-life care, ensuring the physical and emotional well-being of patients and their families.
  • Deliver expert pain and symptom management, offering comfort and support during challenging times.
  • Collaborate closely with our interdisciplinary team, local physicians, and community providers to optimize the quality of life for patients.
