About The Position

The Care Team is seeking a part-time Registered Nurse to cover Wayne County who will support hospice patients receiving care in their homes or facility settings by providing skilled nursing care and coordinated, compassionate end-of-life support in partnership with the interdisciplinary team. Schedule, Hours & Territory • Standard operating hours are Monday through Friday, 8:30 AM to 4:30 PM. Requirements

• Licensed or registered in the state in which currently practices
• Must pass a criminal background check & MVR check.
• Completed health statement acknowledging ability to perform the duties of the position.
• Valid state drivers license.
• Proof of current automobile insurance.
• Current CPR card; TB testing per agency policy

Responsibilities

• Conduct comprehensive patient assessments and develop individualized plans of care
• Provide skilled nursing care in accordance with hospice standards and physician orders
• Coordinate care with physicians, caregivers, and interdisciplinary team members
• Educate patients and families on symptom management, disease progression, and end-of-life care
• Document assessments, interventions, and care plans accurately and timely in the EMR HCHB (Homecare Homebase)
• Participate in interdisciplinary team meetings and care planning
• Maintains the agency's mission, philosophy and core values.
• Ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety and emergency management.
• Always maintains patient confidentiality, including all HIPAA regulations.
