About The Position

Hospice of the Piedmont is a life-changing, mission-driven organization offering flexibility and a team-based work environment with a supportive management team. We are seeking a PRN RN for our Night/Evening/Weekend Services team in the Charlottesville and surrounding areas who want the opportunity to truly use their nursing skills in an autonomous setting and to make a difference. This Night, Evening, and Weekend (NEWT) Crisis RN will be a versatile nurse with excellent assessment and nursing skills that translate to the bedside. This role is responsible for delivering high-quality patient care through timely and responsible management of patient/family needs by responding to triage telephone calls, making visits as needed, and liaising with the medical team and vendors through the weekends and nights.

Requirements

  • Graduate of an accredited nursing school
  • Licensed as a Registered Nurse in the Commonwealth of Virginia
  • Intermediate proficiency with EMR systems
  • Two years of relevant nursing experience required (relevant includes bedside, geriatric, long-term care, oncology, cardiology, palliative, hospice, ICU, Step Down or any combination of the above)
  • Driver’s license, personal automobile and current automobile liability insurance
  • Strong nursing practice and ability to assess patient/family needs in varied settings.
  • Effective oral/written communication and collaboration within teams.
  • Empathy, emotional maturity, and ability to support terminally ill patients and families under stress.
  • Highly organized, self-motivated, able to manage multiple tasks and prioritize effectively.
  • Skilled in writing comprehensive reports and maintaining confidentiality of patient data.
  • Adaptable to on-call duties, extended hours, and committed to agency policies and continuous learning.

Responsibilities

  • Consistently responds to HOPs answering service via beeper and/or phone to triage calls
  • Maintains log for calls received
  • Makes home visits appropriately to assess patient needs, to provide nursing care needs and emotional support to family, as needed, when patient dies
  • Performs nursing evaluation visit to new admissions and determines eligibility based on agency criteria, as needed, in order to meet with patient and family members
  • Consults with patient’s physician, Hospice staff, and vendors reporting changes in patient’s condition
  • Observes and reports to physician and other personnel changes in patient’s needs and condition which necessitate a change in the plan of treatment
  • Provides those services requiring specialized nursing skill in accordance with agency guidelines
  • Practices appropriate documentation in a timely manner, using clinical progress notes for nursing services delivered and completes verbal order form in a timely manner
  • Confers with supervisor on call for additional information and guidance, as needed, keeping supervisor informed of any problems that may challenge agency policy
  • Obtains report from weekend on call nurse to prevent any lapse in continuity of on call availability, relaying needed information the next morning to primary staff/supervisor
  • Participates in taking on-call responsibilities as necessary including weekends and evenings
  • Participates in implementing an supporting agency policies and procedures
  • Adheres to safety precautions when planning and making home visits
  • Ability to drive long distances.
  • Due to the nature of services provided, it may be necessary at times for employees to work beyond scheduled work hours to ensure adequate patient care.
