About The Position

At Pathways Hospice (Newest Start Up), our associates are our most valuable asset. We are located in Houston, TX (Harris County). Ultimately, we will serve the vast majority of Harris County initially, our service will start in the East and Southeast Region of Houston at initial phase of service. (Baytown, LaPorte, Clear Lake, etc.) We are excited to be able to contribute to this grow in this amazing healthcare direct-community with our newly formed company, contributing to all patients with a continuum of care and long-term care services. It is our goal to DELIVER excellent and patient experiences to every patient and resident family member. Hospice Registered Nurse (RN) PRN Service Area for Coverage: Northwest & Southwest Houston

Requirements

• Valid RN license in the State of Texas.
• Hospice experience (at least 1 year).
• Knowledge of home health regulations and general healthcare compliance.
• Excellent communication and people skills are essential.
• Ability to work independently and as part of a team.

Nice To Haves

• Experience with Home Care Home Base (HCHB) EMR (preferred but not required).

Responsibilities

• Oversee nursing care team.
• Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.
• Night and Weekends are required per hospice needs of patients
