About The Position

The Registered Nurse Case Manager (RN) at Brighton Hospice plans, organizes, and directs hospice care, emphasizing community health education and experience. The professional nurse builds from community resources to plan and direct services that meet the needs of individuals and families within their homes and communities. Responsibilities include conducting comprehensive assessments of patients and families, coordinating patient care, and providing professional nursing care utilizing all elements of the nursing process. The RN is responsible for developing and initiating care plans, administering medications, and providing health care instructions to patients and families. The role requires completing a minimum number of weekly visits as established by the Office Administrator.

Requirements

  • Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice.
  • Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
  • Ability to work closely with patients on a 1-1 level while receiving strong support from a patient care team.
  • Can work independently and manage day-to-day activities including phone, email, and written communication.
  • Understanding of hospice rules and regulations.
  • Time management skills to manage visits, documentation, and meetings.
  • Ability to learn an electronic medical records system.
  • Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance.

Responsibilities

  • Completes an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs.
  • Provides a complete physical assessment and history of current and previous illness(es).
  • Completes initial evaluations, consents, and admissions for patients.
  • Responsible for coordinating patient care for assigned caseload.
  • Provides professional nursing care by utilizing all elements of nursing process.
  • Assesses and evaluates patient’s status by writing and initiating plan of care.
  • Regularly re-evaluates patient and family/caregiver needs.
  • Participates in revising the plan of care as necessary.
  • Initiates the plan of care and makes necessary revisions as patient status and needs change.
  • Uses health assessment data to determine nursing diagnosis.
  • Develops a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions.
  • Includes the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures.
  • Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
  • Counsels the patient and family in meeting nursing and related needs.
  • Provides health care instructions to the patient as appropriate per assessment and plan.
  • Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optimal comfort care.
  • Must complete weekly visit minimum as established by Office Administrator.
