About The Position
Providence at Home is a growing division of Providence Life Services, which provides a continuum of healthcare, focused on senior living. Providence Hospice is seeking a RN Hospice Case Manager, Registry in St. John, IN. We are looking for a compassionate RN to join our established, experienced hospice team. We provide exceptional patient centered care to patients and families facing end of life. Schedule Available: As needed basis. On-Call rotation is a plus. Requirements
- Active Registered Nurse (RN) License in the state of Indiana.
- At least 1 year of RN experience, preferably in Hospice or Home Health.
- Drivers License in good standing.
Nice To Haves
- On-Call rotation is a plus.
Responsibilities
- Provide skilled nursing care
- Provide case management services
- Conduct initial assessments of new patients
- Travel to patients wherever they reside: Home, nursing facility etc.
