About The Position

Per Diem Hospice Registered Nurse (RN) Liaison - Capital Region The Community Hospice has a rewarding opportunity for a RN to work as a part of our Hospice Intake RN Team serving as a Liaison and Hospice RN in local hospitals (across the Capital Region). Position Highlights: NO WEEKENDS, NO ONCALL National pre-taxed mileage reimbursement Meetings and charting conducted via supplied lap top Comprehensive orientation provided

Requirements

• A current license to practice as a Registered Nurse in the State of New York
• Associates Degree in Nursing, BSN preferred
• 1 year experience as an RN required, preferably in Hospice/Home Care or acute care.
• The RN must be able to communicate effectively, verbally and written
• Valid Driver's License and Reliable vehicle
• High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers

Nice To Haves

• Hospice or RN Liaison experience a plus but not required.
• Understanding of Hospice principles preferred

Responsibilities

• Evaluates patients for eligibility and interest in hospice services, describes the support and care available from hospice along with the benefits of hospice care.
• Coordinates with the hospital providers and our hospice medical directors to confirm eligibility for hospice services.
• Gathers information and ensures a smooth handoff to the hospice RN admitting the patient/family to services.
• Admits to hospice inpatient services upon eligibility, develops a hospice plan of care and collaborates with the hospice interdisciplinary team and the acute care team.
• Works closely with the hospital case management department.
• Needs are generally weekdays and might be some weekend availability. Hours would be between 8am to 4:30pm.
