About The Position

The Hospice Registered Nurse Case Manager at Peaceful Purpose Hospice is responsible for managing the care of hospice patients in several counties, ensuring quality care and support for patients and their families during the end-of-life journey. This role involves regular assessments, developing care plans, and collaborating with interdisciplinary teams to provide comprehensive hospice services.

Requirements

• Currently licensed as a Registered Nurse in Georgia.
• One year of hospice or home health experience.
• Minimum 2 years of experience as a Registered Nurse.
• Reliable transportation with appropriate insurance coverage.
• Good communication skills (verbal and written).
• BLS Certification (Required).

Nice To Haves

• Experience in hospice care (2 years preferred).

Responsibilities

• Regularly assesses and reassesses the nursing needs of the Hospice patient.
• Provides Hospice nursing services, treatments, and preventive procedures.
• Initiates nursing procedures appropriate for the patient's Hospice care and safety.
• Observes signs and symptoms and reports to the physician and IDT members any unexpected changes in the patient's physical or emotional condition.
• Teaches, supervises, and counsels the Hospice patient and family members about providing care for the patient.
• Develops and re-evaluates the patient/family care plan in conjunction with IDT to meet needs and maintain continuity of care.
• Attends team conferences.
• Works with the marketing team to educate the community about hospice services.
