At Lifted Hospice, our mission is simple yet powerful: Helping people live life to the fullest. We serve the communities where we live and work by delivering compassionate, clinically excellent end-of-life care. With a commitment to dignity, comfort, and quality of life, we walk alongside patients and their families—meeting them where they are, when they need it most. What makes us different is how we show up. We care for patients and families like they’re our own—and we support each other the same way. At Lifted Hospice, you’ll be part of a culture built on trust, unity, and joy. We serve from a place of favor, and we never forget the sacredness of what we do. At Lifted Hospice, our RN Case Managers play a vital role—combining clinical excellence with genuine compassion to provide care that honors life and brings comfort when it matters most. As an RN Case Manager, you’ll deliver personalized nursing care in the patient’s home or care setting. You’ll collaborate closely with the interdisciplinary team to build and follow care plans that reflect the patient’s goals, support their family, and meet the highest standards of clinical, ethical, and emotional care. You’ll also educate and equip families to help them understand what to expect, how to provide comfort, and how to feel confident in their role during a sacred season of life. As a Registered Nurse (RN) Case Manager at Lifted Hospice, you’ll provide expert care and steady support for patients and families facing end-of-life. You’ll lead the bedside experience—managing symptoms, guiding care, and helping families feel confident and supported.
