About The Position

The Registered Nurse Hospice Case Manager provides end-of-life care to patients with a terminal diagnosis needing hospice services in their home or community setting. This position will oversee the development, implementation and evaluation of individual client care plans. Maintains an ongoing responsibility for an assigned caseload. Primary location will be out of our St. Paul location but might have some patients in St. Louis Park depending on census.

Requirements

Graduate of an accredited school of nursing required.
Must be licensed as a Registered Nurse in the State of Minnesota and must maintain licensure.
One or more years' recent nursing experience in home care, hospice or public health required.
Current CPR certification and maintenance of certification required.

Responsibilities

Oversee the development, implementation and evaluation of individual client care plans.
Maintains an ongoing responsibility for an assigned caseload.
