About The Position

This Registered Nurse (RN) position is responsible for delivering care and services to a group of patients, visiting them in various settings including private homes, Skilled Nursing Facilities, and assisted living facilities. The RN will act as a contributing member of the interdisciplinary team, offering expertise in psych-social assessments, interventions, and knowledge of community resources.

Requirements

  • Current Registered Nurse (RN) licensure (in good standing) in the state of Ohio
  • Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and aide leadership
  • Can work independently and on your own on a day-to-day basis
  • Understanding of hospice rules and regulations
  • Time management skills to manage visits, documentation, and meetings
  • Ability to learn an electronic medical records system
  • Must be willing to drive with reliable transportation, a valid driver’s license, and auto insurance, to be able to travel across the service area

Responsibilities

  • Provide care and services for a group of patients
  • Visit patients where they reside, including private homes, Skilled Nursing Facilities, and assisted living facilities
  • Contribute to the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources
  • Work closely with patients on a 1-1 level
  • Work independently on a day-to-day basis with activities including phone, email, and written communication
  • Attend meetings with a variety of healthcare professionals
  • Maintain an understanding of hospice rules and regulations
  • Manage visits, documentation, and meetings (e.g., schedule visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns)
