Job Description: Hospice Registered Nurse (RN) Case Manager Field-Based | Full-Time Eaton County At Elara Caring , we believe the best care happens where patients feel most comfortable—at home. Requirements

  • Hospice experience is highly preferred
  • Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Bachelor’s degree)
  • Current, unrestricted State RN license
  • Minimum of 1 year of clinical nursing experience
  • Ability to travel within the service area (approximately 50%)
  • Ability to lift 50–100 lbs and perform hands-on patient care
  • Valid driver’s license, auto insurance, and reliable transportation

Nice To Haves

  • Hospice experience is highly preferred

Responsibilities

  • Conduct comprehensive assessments addressing physical, psychosocial, emotional, and spiritual needs
  • Develop, implement, and coordinate individualized interdisciplinary plans of care
  • Collaborate closely with physicians, Clinical Supervisors, and the hospice care team to ensure continuity of care
  • Educate patients, families, and caregivers on hospice philosophy, pain management, and symptom control
  • Provide compassionate support at time of death and complete bereavement assessments
  • Communicate changes in patient condition and environmental concerns promptly and effectively
  • Serve as a clinical resource and educator for staff in alternate care settings such as nursing homes and assisted living communities
