About The Position

Coordinate hospice care by working with other team members as well as with the patient's family members to ensure needs are met. Evaluate the effectiveness of hospice services to the patient and family on an ongoing basis. Perform evaluations for hospice care along with informational visits as needed. Consult with the attending physician and hospice medical director concerning changes in patient condition requiring order changes and/or changes in the plan of care. Submit complete, accurate, and relevant clinical notes promptly. Participate in IDG and case conferences Performs on-call duties per policy and need Conducts an initial and ongoing comprehensive assessment of the patients Develop and implement the initial plan of care and work with other members of the IDG in completing and maintaining the plan of care. Supervise and evaluate the care given by the hospice Aide at a minimum of once every 14 days or as needed. Provide care plan to Hospice Aide and LPN as part of the IDG team. Perform death visits per company policy Participate in patient's discharge planning process Why work with Care 360 Hospice:

Requirements

  • Licensed Registered Nurse
  • BLS Certified
  • Valid driver's License and Auto insurance Coverage
  • Reliable transportation
  • A minimum of 1 year of Clinical experience
