About The Position

The Hospice Registered Nurse Case Manager coordinates hospice care by collaborating with team members and patient families to ensure patient needs are met. This role involves evaluating the effectiveness of hospice services, performing initial evaluations and informational visits, and consulting with the attending physician and hospice medical director regarding changes in patient condition and care plans. The RNCM is responsible for submitting accurate clinical notes, participating in interdisciplinary group (IDG) and case conferences, and performing on-call duties. Key responsibilities also include conducting comprehensive patient assessments, developing and implementing initial care plans, supervising hospice aides, providing care plans to the IDG team, performing death visits, and participating in patient discharge planning.

Requirements

  • Licensed Registered Nurse
  • BLS Certified
  • Valid Drivers License and Auto insurance Coverage
  • Reliable transportation
  • A minimum of 1 year of Clinical experience

Responsibilities

  • Coordinate hospice care by working with other team members as well as with the patient's family members to ensure needs are met
  • Evaluate the effectiveness of hospice services to the patient and family on an ongoing basis
  • Perform evaluations for hospice care along with informational visits as needed
  • Consult with the attending physician and hospice medical director concerning changes in patient condition requiring order changes and/or changes in the plan of care
  • Submit complete, accurate, and relevant clinical notes promptly
  • Participate in IDG and case conferences
  • Performs on-call duties per policy and need
  • Conducts an initial and ongoing comprehensive assessment of the patients
  • Develop and implement the initial plan of care and work with other members of the IDG in completing and maintaining the plan of care
  • Supervise and evaluate the care given by the hospice Aide at a minimum of once every 14 days or as needed
  • Provide care plan to Hospice Aide and LPN as part of the IDG team
  • Perform death visits per company policy
  • Participate in patient's discharge planning process
