About The Position

The Care Team is looking for a passionate nurse to join our Flint team as a Registered Nurse Case Manager (RNCM). The Registered Nurse Case Manager (RNCM) supports hospice patients receiving care in their homes or facility settings by providing skilled nursing care and coordinated, compassionate end-of-life support in partnership with the interdisciplinary team. Schedule, Hours & Territory • Standard operating hours are Monday through Friday, 8:30 AM to 4:30 PM • Territory: Genesee County • RN Case Managers may participate in an on-call rotation with additional pay • Some branches offer dedicated on-call staff, resulting in backup on-call coverage only What We Offer • Strong interdisciplinary team collaboration • Supportive clinical leadership and resources • Clear expectations and consistent workflows • Environment focused on quality, compliance, and patient-centered care

Requirements

  • Licensed or registered in the state in which currently practices
  • Must pass a criminal background check & MVR check.
  • Completed health statement acknowledging ability to perform the duties of the position.
  • Valid state drivers license.
  • Proof of current automobile insurance.
  • Current CPR card; TB testing per agency policy

Responsibilities

  • Conduct comprehensive patient assessments and develop individualized plans of care
  • Provide skilled nursing care in accordance with hospice standards and physician orders
  • Coordinate care with physicians, caregivers, and interdisciplinary team members
  • Educate patients and families on symptom management, disease progression, and end-of-life care
  • Document assessments, interventions, and care plans accurately and in a timely manner in the EMR HCHB (Homecare Homebase)
  • Participate in interdisciplinary team meetings and care planning
  • Support after-hours patient needs as part of an on-call rotation when applicable
