About The Position

Hospice of Chattanooga is seeking a Full Time Hospice RN Case Manager (RNCM) to join their dedicated nursing team, providing exceptional end-of-life care throughout the community in Bradley and Polk County. In this rewarding role, the RNCM will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. As a valued member of the interdisciplinary care team, the RNCM will help ensure every patient receives The Best Care Possible, grounded in compassion, clinical excellence, and connection. Requirements

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting.
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

• Partner closely with our Clinical Director to assess the eligibility and suitability of clients for our specialized hospice services.
