  • Provide care and services for a group of patients.
  • Visit patients where they reside, including private homes, Skilled Nursing Facilities, and assisted living facilities.
  • Contribute as a member of the interdisciplinary team, providing expertise regarding psych-social assessments and interventions.
  • Utilize knowledge of community resources to assist patients and families.
  • Current Registered Nurse (RN) licensure (in good standing) in the state of Ohio.
  • Ability to work well within a team environment and collaborate daily with nurses, social workers, chaplains, volunteers, and aide leadership.
  • Strong communication skills for 1-1 interactions with patients and collaboration with healthcare professionals.
  • Ability to work independently and manage day-to-day activities including phone, email, and written communication.
  • Time management skills to manage visits, documentation, and meetings effectively.
  • Ability to learn an electronic medical records system.
  • Willingness to drive with reliable transportation, a valid driver’s license, and auto insurance.
