About The Position

RN Workflow Specialist (Remote) – (Hospice Quality Assurance RN) Remote | Registered Nurse (RN) | Hospice Job Overview Puget Sound Hospice is hiring a Remote Hospice RN Workflow Specialist (Hospice Quality Assurance Compliance RN) to support hospice clinical workflows in Homecare Homebase (HCHB) . This role focuses on hospice documentation accuracy, workflow completion, and Medicare compliance across admissions, recertifications, discharges, and physician orders. Candidates must hold a Washington RN license or Compact license and be available to work Pacific Time (PST) hours.

Requirements

  • Graduate of an accredited school of professional nursing
  • Current, unrestricted Registered Nurse (RN) license in Washington State OR Nurse Licensure Compact (NLC) license
  • Minimum one (1) year of hospice nursing experience
  • Experience with Medicare hospice documentation and workflows
  • Homecare Homebase (HCHB) experience required
  • Must be available to work Pacific Time (PST) hours
  • Remote position requiring reliable internet access and a HIPAA‑compliant home workspace

Responsibilities

  • Hospice Workflow & Documentation Oversight Review Hospice Start of Care (SOC) evaluations for completeness, accuracy, and regulatory compliance
  • Verify admission documentation in Homecare Homebase (HCHB) , including visit creation and visit frequencies
  • Review, edit, and verify Initial Plans of Care to ensure alignment with physician orders and hospice eligibility requirements
  • Review, edit, and lock HOPE (Hospice Outcomes & Patient Evaluation) at admission and discharge in accordance with CMS requirements
  • Create and manage Certificates of Terminal Illness (CTIs)
  • Recertification, Orders & Compliance Review, edit, and approve hospice recertifications
  • Review and approve physician orders
  • Monitor visit coordination notes across all disciplines
  • Ensure recertification and discharge workflows are completed accurately and on time
  • Serve as a clinical quality checkpoint to support survey readiness and audit compliance
  • Collaboration & Systems Use Collaborate with intake teams, clinical leadership, and IDG members
  • Support efficient patient flow and documentation integrity across one or more hospice locations
  • Utilize Homecare Homebase (HCHB) and other approved technologies to complete workflow and quality functions
