About the position

The Hospice QA Nurse is responsible for ensuring that hospice care provided to patients meets high-quality standards. This role involves conducting reviews of clinical practices, policies, and procedures to ensure compliance with state and federal regulations, as well as accrediting bodies. The Hospice QA Nurse collaborates with interdisciplinary team members to promote optimal patient care and safety.

Responsibilities

  • Conduct comprehensive chart audits to assess the quality and completeness of patient records.
  • Ensure compliance with state and federal regulations, accreditation requirements, and agency policies.
  • Monitor documentation of patient care, ensuring it meets regulatory and hospice care standards.
  • Review and evaluate clinical processes and practices to identify areas for improvement.
  • Assist with the development and implementation of performance improvement plans and corrective actions.
  • Perform case reviews of patient care provided by the interdisciplinary team to ensure quality standards.
  • Evaluate care plans and interventions for appropriateness, timeliness, and patient-centeredness.
  • Review medication management and administration practices to ensure safety and effectiveness.
  • Identify trends and patterns in patient outcomes to recommend improvements or corrective measures.
  • Provide ongoing education and training for clinical staff regarding regulatory compliance, documentation standards, and best practices.
  • Facilitate the development and delivery of quality improvement and infection control education.
  • Assist in developing training materials and tools to enhance staff knowledge of quality standards.
  • Lead or participate in the development and implementation of performance improvement initiatives.
  • Track and analyze data related to patient care quality, including patient satisfaction, adverse events, and clinical outcomes.
  • Prepare and present regular reports on quality metrics to management, highlighting areas of concern and recommendations for improvement.
  • Ensure that patient care meets the highest ethical and professional standards.
  • Act as a resource for patients and families regarding quality care expectations and concerns.
  • Promote patient-centered care and work closely with the team to ensure the patients' needs are being met.
  • Work closely with the Hospice Director, Clinical Manager, and other team members to develop and review policies and procedures.
  • Collaborate with other healthcare providers to ensure continuous quality care for hospice patients.
  • Participate in team meetings and contribute to the development of care plans.

Requirements

  • Nurse (RN/LVN) with a current license to practice in the state.
  • Minimum of 2 years of clinical nursing experience, with at least 1 year in hospice or palliative care.
  • Strong understanding of hospice care regulations, compliance standards, and quality assurance practices.
  • Excellent clinical assessment and critical thinking skills.
  • Strong attention to detail, organizational skills, and the ability to handle multiple priorities.
  • Proficient in using electronic health records (EHR) and other healthcare management software.
  • Effective communication and interpersonal skills for collaboration with the healthcare team and patient families.

Nice-to-haves

  • Certification in hospice and palliative care (CHPN) is preferred.
  • Previous experience in a quality assurance or compliance role in a healthcare setting.
