About The Position

The Hospice PRN Registered Nurse Case Manager is responsible for providing skilled nursing care to clients in their homes, coordinating care with an interdisciplinary team, and ensuring effective communication with patients, families, and referring agencies. This part-time role requires flexibility in scheduling and the ability to respond to emergency calls as needed.

Requirements

• Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse.
• Current State Driver's License.
• Two years of experience as a Registered Nurse in a clinical care setting, with hospice experience required.
• Nursing skills as defined by generally accepted standards of practice.
• Good interpersonal skills.
• Proof of current CPR certification.
• Ability to read, write, and comprehend English.

Responsibilities

• Administer skilled nursing care for clients in their place of residence.
